If not, you need to fill out the AUP Form, signed by you and your parent/guardians and hand in by Friday, February 19th!!  This form with a cover letter was sent in the summer to your home.

If you do not hand it in, you will not have access to the Internet or have your picture in the yearbook!   Accounts will be shut down on February 22nd!

PICK UP DATE:  MARCH 11, 2010

There are two ways to order:
1. View the online catalog and place orders by going to www.pamperedchef.biz/davinamessics.   Click on "Order Products," choose option 1, and then enter Nazareth as the organization.
2. Browse through a catalog that can be picked up in the school nurse's office and use this order form.  Feel free to make copies as necessary.

Shipping & handling options:
1. Orders to be picked up at the school pay a $4.25 flat S&H rate.
2. If you would like your order shipped directly to you, you may pay a direct ship rate according to this chart on the order form

Please turn in non-online orders with payments by February 18th to Mrs. Hoch in the high school nurse’s office. Be sure the student's name is on each order form.  

Orders will be available for pick-up on March 11th from 6pm - 9pm in the LGI Lobby, unless you are having the products shipped directly to your home.
